TESTIMONIAL RELEASE

By signing/ consenting through affirmative action to this Testimonial Release (“Release”), you are authorizing Sophie Eva Allison to use and disclose your name, photograph and testimonial (collectively referred to as “Testimonial” in its marketing and public relations efforts.  You further acknowledge that your Testimonial may be distributed to the media, third parties, the public, and entities that may be involved in our marketing and public relations efforts.

You understand and agree that the Testimonial that you submit will become property of Sophie Eva Allison and will not be returned.

CONSENT TO RELEASE

I hereby authorize Sophie Eva Allison, its representatives and employees, the right to copy, exhibit, publish, distribute or otherwise use and disclose my Testimonial and any information contained in the testimonial in various marketing initiatives, for any other marketing and public relations efforts or for any other lawful purpose.  These statements may be used in printed publications, multimedia presentations, on websites, electronic format or in any other media.  I approve the disclosure by Sophie Eva Allison of my Testimonial and other information to the media, the public and other individuals and entities that may be involved in the marketing and public relations efforts of Sophie Eva Allison

I agree that I will make no monetary or other claim against Sophie Eva Allison for the use of the Testimonial.  I waive any right to inspect or approve the finished product where my likeness or my testimony appears. I hereby hold harmless and release Sophie Eva Allison from all claims, demands, and causes of actions for damages of any kind based on the use of my Testimonial or information provided within the testimonial.  This binds my heirs, representatives, executors, administrators or any other persons acting on my behalf or on behalf of my estate.

I understand and agree that the Testimonial and information I submitted becomes the property of Sophie Eva Allison.

I signed/ consented to this Release voluntarily. I also warrant that I am at least 18 years of age and have the competency to enter a contract in my own name.  I have read this Release before signing/ consenting through affirmative action the same and I fully understand the contents, meaning, impact and consequences of this Release.
